The following forms are required for all 7-12 grade students participating in sports in the Anamosa Community School District. These forms must be submitted to the school office before the first day of practice. Your child will NOT be allowed to practice until all forms have been received.
Feel free to download and print these forms. There are extra forms available for pick up at the Anamosa High School and Administrative Offices. Physical forms are also available at the local doctors office.
